I just went this past thanksgiving. Not much to do on the islands, but on the main land, jungle tours are a lot of fun. Also just out side of belize city are some springs that you can float through on tube or canoe which are really cool as the river run right through caverns.
When I was on the islands, I did nothing but bonefish. Great fishing for permit and cuda too.
I'm with Dan, my absolute two favorite things to do when I was there about a year and a half ago was horseback riding through the rainforest and tubing through the caves. It was sort of bizarre. We found a cowboy from Montana who had retired there and did organized tours of his property as well as the government owned property next to his. Tons of fun... you can also check out the official Belize Tourism Board website to get some other great ideas. I hope to go back someday soon. It has been my favorite destination outside the US...
Our trip last August / September took us to PG in Toledo District - a little off the beaten path, but unbeatable for off-highway exploration and if you want to immerse yourself completely in the various cultures on offer.
