My former employer was the midwest distributor. We purchased $20K worth of product from them in June of '09 and received only $13K of product in July. By October they stopped returning our calls and disappeared. We heard the owner was sent to Iraq so we decided to wait for him to come back before asking for the product owed or a check for the difference. Later we were told by someone in the company that Safari Gards owner was no longer in the military but a civilian contractor that had elected to pursue contract work in Iraq.
It's to bad because they built a fine product. Apparently they owe all three of their distributors money so I doubt they'll be back in business again.

Search http://www.mcmaster.com/ for PN: 1723A63 or 1723A64
That is what SG gave me.

For the hoist ring go to:http://www.mcmaster.com/#hoist-rings/=6mr6n4
click standard hoist rings, 3/4"-10 threads, 5000# working load (remember it has a 5:1 safety factor) and pick one you like (probably want the bolt to be at least 1 1/2".

The backing plate SG gave me was just a circle of 1/4" plate with a 3/4" hole in it for the bolt to go through.

You would also need a nut for of the rings, PN: 97135A280
